Temple City Dial-A-Ride Program provides curb to curb transportation service for Temple City residents living within the City limits and approved unincorporated county areas adjacent to Temple City with restrictions, 60 years and over or under 60 years with a physical, psychological or developmental disability certified by a physician.
Applications for Dial-A-Ride are processed at the
Live Oak Park Annex, 10144 Bogue Street
Monday - Friday   8 a.m. - 6 p.m.

Submit your application ONLINE
Applications submitted online must include a copy of CA Driver's License or CA ID for processing. If your Photo ID does not list your current address, please include a copy of a current utility bill.

Email proof of residency and age to:
Unincorporated County Area adjacent to Temple City between Duarte Road to the north, the railroad tracks to the south, San Gabriel Blvd. to the west and 6th Street to the east

Persons under 60 years with a physical, psychological, or developmental disability must complete a Physicians Verification Form and return with application

Proof of residency required with application. If under age 60 and disabled, the Physicians Verification Form is required at time of submitting application.
